What should I do before I come to my first appointment?

If you would like, when you schedule your first appointment you can ask them to send you the New Patient Medical Information Packet. This packet will include the demographics and medical history form, as well as a map to find us, or you can come a few minutes early and fill them out in our office. You also have the option of downloading the New Patient Medical History Form and the New Patient & Yearly Forms from our Patient Forms page to complete at your leisure at home before your appointment. We strongly recommend that you bring all medications that you are taking to every appointment. You will also need some information regarding your family's medical history. You should bring your insurance card and an identification card.

What should I do if I need to cancel my appointment?

Sometimes, it's unavoidable to have to cancel appointments. If you are unable to keep a scheduled appointment, please try to call your provider's office at least 24 hours in advance. Office staff will cancel the appointment and can assist you in rescheduling for another date.

What happens if I miss my appointment?

It is very important to try to keep all scheduled appointments. When you schedule an appointment, the office staff reserves time for you to be seen by your provider and prepares your health records for the visit. We ask that you call your provider's office as soon as you know you are unable to attend your appointment. Not only will this let us reschedule your appointment for another time, but it also frees up time for another patient to be seen.

What happens if I'm late to my appointment?

Please call your provider's office to let them know you will be late to an appointment. Because many patients are scheduled each day, office staff can only wait so long before seeing the next scheduled patient. If you are more than 10 minutes late, the office may need to reschedule your appointment.

What if I'm having a medical emergency or need care after hours?

If you are experiencing a life-threatening emergency, call 911 right away. Do not call your provider first. Make a follow-up call with your provider either within 24 hours, or when your health allows. For non-life-threatening urgent medical care after hours or on weekends, patients of Indiana Care Network should consider a visit to the nearest Urgent Care facility.
